    which in this sentence?

    In his report Dr. Lee advised that his patient had back problems as a result of a fall but his symptoms settled within four weeks thus enabled him to return to work.

    1 - I added commas after "fall" and "weeks". Commas are required when combining sentences with coordinating conjunctions such as "but", and also before dependent clauses.

    2 - What does "settled" mean in this context? My best guess is "improved". If this is correct, then "improved" is a much better word to use.

    I wouldn't use advised that- said/stated/wrote would work better IMO.
