    Can you please correct this

I want to write a Recommendation for terminating one employee who do not work well and not obeying his supervisor- please see below and correct.

Sir, This is to inform you that I have received several complaints about one of our drivers Mr... regarding his poor work performance and disobedience to the supervisors. On(date), Mr...(supervisor) assigned him a task which was not properly carried out by him that ended up a shouting match both of them. Later Mr..(supervisor) decided to transfer him to our antoher location and advised him accordingly but he denied working in our other loaction and asking for a release from this department to another. Since the day he joined, we are receiving many complaints from his supervisor and co-workers about his attitude and disobedience, therefore, I recommend to terminate this employee with immediate effect to avoid further disturbance in the department.

fivejedjon, why you substitute "refusal" for "disobidience"?
Because "disobidience" does not collocate with "supervisors" well?
Or because "disobidience" is a bit extreme?

While adults can disobey orders/instructions, I associate the noun 'disobedience' more with children. This may be a personal feeling.
