rather didn't speak; very vs. really

Hello,

Here's the test question:

I'd rather ___ to me like that in front of the customers.
a) didn't speak b)not speak c) weren't speaking.


I understand the correct verb tense should be the past simple (a), but I think the sentence sounds awkward without a pronoun - in this case "I'd rather YOU didn't speak to me like that...." would be much better. Would you agree with me?

Here's another question:

The fish is ____ delicious.
a) very b) really c) completely


The correct answer is b). I'd pick b) as well as it sounds best. But is a) WRONG? If yes, can you explain why?

I'd love to hear your opinions.
