    Question format

    She wants to go there.

Does she want to go there? or
    Want she go there?
Are they both correct? I saw this in our grammar book.

    Throw away that grammar book.
    Now.
Unless of course you have mis-read 'won't she go there?

    Throw out your grammar book.

You can form a question by inverting the verb with "have." Have you any idea how long until we get to Worcester?

To do it with the verb "to want" will sound decidedly non-native. If you do it, however, at least the correct form of the verb: wants. Actually, just don't to it.
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
