"How are you?" not for the first meeting?

Can't you say "How are you?" for the first meeting with an unknown person? Can you only say "Nice to meet you" or something?
I've seen this in a middle school reference book, which I think is weird.

'How are you?' is commonly heard these days, especially when one is being introduced by a friend to one of their friends.

I don't think this is quite as common in the United States. However, we might say something similar, like, "How's it going?" or "What's up?"
Of course, in more formal situations we still use "How do you do?", which means the same thing as "How are you?"!
