    If you think that telling someone that they are a mess is rude, and you consider yourself a polite person, you could say that you are too polite to tell someone that they have something on their face.
    I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.

    The boy is also implying that the teacher is impolite for mentioning it - and that's why she looks shocked, or taken aback. She's wondering whether she has just been insulted by the boy's remark.
