    2) What information does the required?
    3) what information does the form ask you?
    4) You need to fill in the information on the places you live before in the last 2 years.

    Michael, you need to give your threads more meaningful titles. You use the same one nearly every time. Why not call this one Information on a form?
    1) What it the form about? OK

    2) What information does the is required?

    3) What information does the form ask you for?

    4) You need to give all your addresses during the last two years.
