    None of those are correct. You need verbs in the first 3. #4 is a mess, I don't know what you are trying to do.

    She has never been satisfied with the effort Tom puts into his work.

    She is never satisfied with the effort Tom puts into work. (contented is not a particularly good word choice)

    She is never happy (content) with how hard Tom has been working.

    No, satisfied is probably usually the safer choice.
    Contented is a bit more like a cow.
