Is this saying "his family can get the benefit of the health plan if he earns $1 a year"? Maybe I don't understand the system.
You simply have to be an employee (not a volunteer).
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
Do you mean "the health plan" in the original quote refers to Apple's health plan, not the national health plan?
