Need advice on pronouns, thanks teachers!

I was wondering if it's wrong to write:

During recess, I, Mary and John went to the playground.

    OR should it be:
During recess, Mary, John and I went to the playground.

Should we put the pronoun "I" at the end when there are more than one subjects in the sentence?

    PLUS:

My teacher Miss Smith is kind.
Should we put a comma after "My teacher"?
    i.e.
My teacher, Miss Smith is kind.

Thanks sooooo much for advising, teachers!

1- Put 'I' at the end.
2- When you have two things referring to a common subject, then put one between commas, though many don't.
