    How would I know that?

    They're arguing about her attitude toward his psychological approach or something. When you say "How would I know that?" , does it mean an opinion or presumption? 1 or 2? Is she sure or unsure of the knowing?
    1. Opinion - I'm sure I don't know that (but I'm saying in a mild, euphimistic way not to be offensive)
    2. presumption - I'm not sure I don't know that, maybe I wouldn't know that.

    Mel - How would I know that?

    Juding from the context, I would say it's an opinion as Doc said "You're doing it again = You're not accepting that you're not straight with me".

    He's presuming to read her mind and she is calling him on it.

    She could very well already know what he says she knows, but she has not revealed that she does, so she is calling him on his "mind reading."
