The definition of "put a spin on something" is "to twist a report or story to one's advantage", but in here, I can't get what it means. Mel is trying to get the meaning of a tattoo that appeared on her husband's back from the doctor who is apparently quite pleased with her coming as she is beautiful.
Is this expression related to the husband's tattoo or her appearance?

It's a little difficult to say just from this short dialogue, but I suggest that he means: You know how to throw someone's day slightly out of kilter by introducing something unexpected/unusual.
This might be caused by both her appearance and her story together.
