**Location and spot both have the same meaning, so saying spot is redundant, like saying "location of the accident location."

2) Provide us with your (home) address.
**In identifying your place of residence, you give your address, so that's what you would be asked for.
If you were simply being asked where you are at that time, then it's just "provide us with your location."
