Your first and third sentences were OK, but the second is not very natural. The # marks other possible positions.

    - I was able to easily swim across this river if I wanted to.
    - He could run a mile in five minutes when he was younger.
    - Jack could reach the s South p Pole before Scott.
    - I rang up several times before I was able to contact him.
    - I was able to get these tickets because I was willing to stand in the queue for several times.
In the first you are talking about a hypothetical possibility. In the third you are talking about something that actually happened

Yes. You would use the word only once, of course, but it could go in any of the spots marked with the # sign.
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.

    Another question :

Why we can say : I am in such a hurry, I don't often have time for breakfast.
But we can't say : We buy things we don't often really need.

We could, but it would be far more likely with 'sometimes/occasionally/often/etc' modifying 'but'.
