1. Does this "as confused as ever" mean "you've been always confused all the time", not "you're most confused than ever"?
2. Does this "As smart and dedicated as you may be" mean "however smart and dedicated you may be", a concessive clause?

No, it means you are as confused as the most confused time for you in the past, hence, at a height of confusion.

Basically, yes. Really he could also say regardless of how smart and dedicated you may be....

I would say that the "as confused as ever" here specifically refers to the most confused the person has been about their career choices. They may have been more confused about other subjects at some point in their life, but as far as their career choice is concerned, they are as confused as they have ever been so far about that topic.
