Is it possible to use the past tense with always or never?

Does "I could to do" make sense? Don’t we need a verb in the base form after a modal verb?

Is there any reason why you always put a comma after 'Thank you'?

Thank you for your question, CS. I do receive emails such the above one from native English speakers using a comma after "Thank you." I think it is a style. But if it goes against your wishes, I will not use that style any more.
