    Hospitals have been investigated by Government authorities.

    In the above sentence, what is the "subject".

    The grammatical subject of the passive verb 'have been investigated' is 'hospitals'. It makes no difference that the 'performers' of the investigating are 'the government authorities' - it is the hospitals that are 'being investigated'.
