    The patient's insight and judgment ( appear appears ) to be poor.

    Could you let me know the correct answer and the rule for appear and appears?

    The rule is not about the verb; the rule is about subject-verb agreement. The subject has two items so it is plural; therefore the verb must be plural "appear".
