This is an oft repeated phase and can be literal in meaning. Who will provide the next meal? Where will the food and water come from? Consider the case of the people caught in the Katrina hurricane in Louisiana in 2005. The people in the impacted area seriously did not know if the city, the county, the state, the federal government, the Red Cross, or some other source would provide food and water.Some contexts here.
If it had been "Many people in the world don't know when their next meal is coming.", it wouldn't have misled charlie or me.
