    happen out of ....

    Can one say:
    1-Most deadly accidents happen out of cities.

    instead of:
    2-Most deadly accident do not happen inside cities.

    I don't find '1' particularly natural, but I don't find an elegant equivalent. 'Outside cities' would make it sound as if they happened close to the cities.

    Your #2 is fine. As would be "outside of cities."

    Nothing in "outside of" implies that it happens nearby.

    I'd just use outside rather than outside of.
