1. John went out to smoke.
2. John went out smoking.

In sentence 1, I think it means that John went out because he wanted to smoke. Does sentence 2 have a similiar meaning?

No. It means that he was smoking when he went out.

#1. Says that he went out to do the act of smoking.
#2. Says that he was doing the act of smoking as he went out.
