    understand after had

    I had not "understand" what he said, so I asked him to repeat the directions.

    Use of understand after had is grammatically correct here?

    No, it isn't. It should be "understood".

    "I had not understood…" (sounds a bit strange though) or, "I did not understand…"

    'I had not "understand" what he said', in your link, vaibhavmaskar, is clearly a typo for 'I did not understand...'

    Use of understand after had is grammatically correct here? That is not the correct way to ask a question.
