I want to form these sentences in questions:
1)You appreciate being helped in this way by Sam.
2)You didn't want to be disturbed.
3)The house seems to have been painted recently.
4)I remember having been shown that museum by John.

Indeed we do want you to try first, but there are so many ways you make these into questions.
For example, for #1:
What did Sam do?
What did you appreciate?
What did you appreciate Sam doing?
Who helped you?
Who do you appreciate having helped you?
How do you feel about Sam's helping you?
How do you feel about having been helped?
How do you feel about what Sam did?

I understand the question to be such that there is only one way to make it a question:
1)"You appreciate being helped in this way by Sam." => "Do you appreciate being helped in this way by Sam?"

You're kidding, right? Even if this is not homework, I've given you a template. Even without that, you simply need to look up how to make a question from a statement in English.
