    I thought that

    I'm not sure which tense (Past Simple/Conitnuous or Past Perfect Simple/Continuous) I should use after the phrase "I thought that":
    I thought that he was spending/had been spending his time with his friends and forgot/had forgotten about our meeting. OR
    I thought that she had/had had an accident and that she was/had been in trouble.

    There are various possibilities within the sentences you have suggested.

    I thought he was spending time with his friends and had forgotten about our meeting.
    I thought he had been spending time with his friends and had forgotten about our meeting.
    I thought he was spending time with his friends and forgot about our meeting.

    I thought that she had had an accident and was in trouble.
    I thought that she had had an accident and [that she] had been in trouble.

    The only one of your suggestions that doesn't work is "I thought that she had an accident".
    Remember - if you don't use correct capitalisation, punctuation and spacing, anything you write will be incorrect.
