    What is the correct sentence?

    It doesn't sound very natural either way, but if you use the plural subject 'medicines', you must use the plural verb 'are'.
    Otherwise, how to say it better depends on what exactly you want to say.

    I mean that medicines that patients take in a hospital are strong.

    How can I say this using the context of another sentence?

    If it were true, it would be natural to say, "The medicines they give you in hospital are very strong. The medicine they give to outpatients is weaker."
