    "How are you?" *can* be a real question, if the person asking knows you have recently been unwell or had some sort of issue.

    However, in most cases, in the U.S., "How are you?" is not an actual inquiry about your state of physical or mental health. Unless you are clearly suffereing from something, the only expected answer is something like "Fine, thanks. You?" or "Good, thanks. You?" or even a "Howareya?" response without even saying how you are.
    I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
