What is the proper/polite/common way to ask for name?

QA) If a person introduce himself and you cannot hear clearly, what is the proper way to ask again?

QB) If a person contacted you (email/talk to you in person), but you cannot remember him, what is the proper way to ask how does he know you?

QC) When a person is exchanging contact with you, do you ask him how to spell it? Do you ask for first name? What is the proper way to ask it?

Excuse me, I am trying to recall if we have spoken before/ you have emailed me before.

I would suggest that "Come again?" might sound too abrupt in this circumstance.
Another possibility for (A) is "Sorry, I didn't catch your name".

For the spelling, I would say "How do you spell that?" as soon as they have said their name and I want to write it down.

When the person only tells you his first name, do you commonly ask for last name? I mean sometimes you cannot remember that John, or sometimes you just want to put the last name when adding into your contact list.

Is it rude to ask for last name? Just ask "What is your last name?" will do?

There's no reason why you can't say something like: "And what's your last name, John, so I can add you to my contacts?".

It depends on the situation. If you're meeting someone at a party, first names usually suffice. If it's a business meeting or you are meeting a business contact for the first time, asking for their surname would be reasonable.
