*Not a teacher.
My suggestions:
He takes a shower.
[STRIKE]Did[/STRIKE]Does he take a shower?

He [STRIKE]have[/STRIKE]has a shower.
[STRIKE][STRIKE]Have[/STRIKE]Has he [STRIKE]had[/STRIKE] a shower?[/STRIKE]

Are they [STRIKE]correctly[/STRIKE]correct?

The first is grammatical, but it is usually followed by something else, such as "every day".
The second is fine.
The third is incorrect. It would be OK with "had" instead of "have".
The fourth is incorrect. It would be OK with "has" instead of "have".

Your last sentence should be "Are they correct?"

For asking whether he is showering or not.
Do I say, "Is he showering?" or "Does he take a shower?"

If he is showering at this time, use the first. Since pretty much everybody showers or bathes, the second question is uncommon.
