    My sister thinks we're ugly in those pictures. I say:

    I don't think that that's possible.

    VS

    I don't think that's possible.

    What's the difference here? What's the double that do? Is it just for effect?

    The first 'that' is a conjunction (or relatiive pronoun) which, as you know, can sometimes be left out in English, as in your second example.
    The second 'that' is a determiner.

    Yes, they mean exactly the same thing. As Raymott said, the first 'that' can be omitted, and there is no resultant change in meaning.
