If you are adding someone to a group conversation or a conference call, you can say "I am patching Ram in".

Only if he has an injury that requires a bandage or a plaster and for some reason he can't get to a doctor. You could only use definition #2 with your word construction.

Is it necessary to add "in" at last?

Yes, you "patch someone in" to a conference call. It's a phrasal verb.

The OneLook site searches dozens of free online dictionaries which are regularly updated. Dictionaries in book form takes years to do this, involve you in needless expense, and are obsolete almost as soon as they appear on the shelves.
