Sentence analysis - help needed

Hi, I'm working on an analysis for my syntax class and I have no idea how to analyze the four underlined elements of the sentence:

Presented as a psychological thriller, this text tells the story of an ordinary young black woman, who, thought to be insane, has spent fourteen years in several mental hospitals trying to come to terms with her family past, manifested outwardly by metaphysical episodes, embodied visions, and haunting reincarnations.

In my opinion, the sentence is overly complex.

As written, the first is an adjectival participial phrase modifying the noun "text". The second is an adjectival participial phrase modifying the noun "woman". The third is an adverbial participial phrase modifying the verb "spent". The fourth is an adjectival participial phrase modifying the noun "past".
