    Can I use both ''What/Who is he to you'' to mean what's your relationship with him?

    I find them both natural, but would probably say "Who..." myself.

    I agree with Rover, so this may be a BrE/NAmE difference.

    Either one works for me. I'm not sure if I even have a preference between the two, I probably use both.

    It looks like it. I'd always use "what".
