    "What insurance this patient has had since then?"

    "What insurance company this patient has been with since then?"

    I would use these:
    "What insurance did this patient have since then?"
    "What insurance company did this patient be with since then?"

    No. Move 'has' to come before 'this patient'.

    This is wrong. We do not form question like this with the verb be. We invert.
