    Re: Address

If I understand your question, no heading is necessary. It will be obvious that it is your address.

    Re: Address

If it is in the body of the email, I was use:

    Please send to:

    Title, Name and Surname
    etc

    Re: Address

I think you should change "was" to "would".

    Re: Address

It is customary to refer to a "shipping address." As opposed to a "billing address."

It is in the center of the letter!

    Re: Address

What country are you sending it to?
    In the US, it's
    [title optional] Full name
    Street address
    City, State, Postal Code
    USA

You would not put, for example
    90210 Beverly Hills CA

    You would put
    Beverly Hills, CA 90210
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.

I mean how should I write 'Address:' or something else in the center of the letter before address itself?
Or I shouldn't write anything? Just without any previous words my address?
With your example it is like -

    Address:

    [title optional] Full name
    Street address
    City, State, Postal Code
    USA

    Or

    Address to send:

    [title optional] Full name
    Street address
    City, State, Postal Code
    USA

    Re: Address

    I would say something like:

    Send to:

    John Doe
    1313 Mockingbird Lane
    Anytown, NY 10034
    USA
