    When children defecate

    (On diapers)
    Did you poop?
Did you poop on your diapers?
    Do you have poop?
Do you have poop on your diapers?
    Did you mess your diapers?

    (On diapers)
Let me get you cleaned up.
    Let's get you cleaned up.
    I'll clean you up.
    I'll wash your butt.
    I'll change your diapers.

    (No diapers)
    Do you need to poop?
Do you need to go to the bathroom?

    (While pooping)
Tell me when you're done so I can clean you up.
Tell me when you're done so I can wash your butt.

That's the only one that is completely unnatural. All the others are possible. Individual families talk about these things in many different ways; there is no standard form.

Note that 'diapers' are normally 'nappies' in BrE.

In BrE, we wipe our bottoms after defecating.
Remember - if you don't use correct capitalisation, punctuation and spacing, anything you write will be incorrect.
