I don't know about Canadian English, but in British English we don't use "born with a silver spoon" without following it with "in his/her mouth". I'm not suggesting that we wouldn't know what it meant if someone used just the first five words, but it would sound unfinished.

It is perfectly fine in Canadian English whether or not the silver spoon is placed in his mouth or not... the meaning is the same.

That's British aristocrats for you- it's not enough to have a silver spoon when you're born; you have to know where it goes. ;-)
