Do I know vs would I know

How do I know if I have Diabetes? And
How would I know if I have or had Diabetes?
please could someone explain to me the grammar for the correct use of do vs would in this type of construction. I thought would is used for imaginary situations.
    Thanks in advance.

Re: Do I know vs would I know

You need to learn how to use quotation marks, so that people know what is part of your sentence and what is not.
"I thought would is used for ..." Wrong
"I thought 'would' is used for ..." Right
"I thought would is used for ..." Right

Re: Do I know vs would I know

When you ask How would I know if I had diabetes? you are considering a hypothetical case; therefore, it's an imaginary situation.

Names of medical conditions are not proper nouns except when they are named after people. Thus you should write (for example) diabetes and essential tremor, but Tourette's syndrome or Lou Gehrig's disease.
