    But once they have decreased, ...

    Sometimes I must take Profinal/Brufen (pills) to relieve my pain. But once they have decreased, the pain gets worse.

    How do I write that part correctly?

    Once the painkillers start to wear off ...
    Remember - if you don't use correct capitalisation, punctuation and spacing, anything you write will be incorrect.

    I suggest:

    When the pain pill starts to wear off, the pain comes back.

    (The pain doesn't get worse. It just returns.)
