Hi,

1- I just wanted to make sure that you are/were OK.
2- I just wanted to make sure if you are/were OK.

I know that I can use 'were' in both examples, but I still want to ask if 'are' is OK or not.

Yes, you could use "are". In the right context, these are all OK:

I just want to make sure you are OK.
I just wanted to make sure you are OK.
I just wanted to make sure you were OK.

Thanks for the answer. Does using 'if' after 'make sure' sound wrong to you?

I would use 'know if' instead of 'make sure if'.
