    I would be thankful if you could give me the required information.
    I would be thankful if I am provided with required piece of information.

    Please tell me which one of the above is correct. [and, here 'please tell me' is followed by a question. Is it correct to put it this way?]

    Use the first. However, I would use "grateful", not "thankful". We tend to use the latter when referring to things that we are happy about on a daily basis - good health, having a job, having an amazing family. I'd also say "necessary information", not "required".

    With the second, if you want to use the first person singular, you'd have to say "I would be grateful if I were provided with ...". There is definitely no need for "piece of".
    Remember - if you don't use correct capitalisation, punctuation and spacing, anything you write will be incorrect.

    The question mark is incorrect because the sentence it ends is not a question. Here's what that statement would look like as a question:

    Will someone please tell me which of the above is correct?

    I am not a teacher.

    It is not followed by a question.
    1. Please tell me the truth.
    2. Please tell me which of the above is correct.
    The underlined parts in 1 and 2 are a noun phrase and a noun clause respectively.
