    Are the following sentences grammatically correct?

    "Kelly smiled after her truly intentions were revealed by her friend Kimberly. She approved the idea with a gesture with her head."

    "True intentions."

    "With a head gesture" is more natural.

    Given that she's showing approval, I'd say "... with a nod [of her head]".
    Remember - if you don't use correct capitalisation, punctuation and spacing, anything you write will be incorrect.
