    Question Is the sentence correct?

    Hi all,

    I want to ask my colleague to confirm that does the information he provided us is correct?

    the sentence as bellow:

    "would you please help to confirm that why does the form you provided us do not include the following items?"

    I would change the first question mark to a full stop because it doesn't look like a question.
