Could anyone please tell me that how to use this sentence properly in imperative"Has your car repaired or Have your car repaired? I am aware of use of "get" but in formal how to say it. What to use have your car or has your car?(Sorry if I made any mistake in writing, I am just a beginner.)

    These sentences are not correct.

If you want to instruct somebody, you could say Get your car repaired.

What do you mean by "formal"? Do you mean in writing? Whom do you want to say this to?

'Have your car repaired' seems fine to me.
