See my changes above. I have a question though - is it a request or a notification? If it's a request, as shown at the top, the body should be something like "I would like to request permission/authorisation to work from home today". If it's a notification, your main sentence is OK but you shouldn't use "Request" at the start.

Note that I have changed your thread title. Titles should include words/phrases from your question and be unique and relevant to the thread.
