I would like to say that the agent in customer care didn't notice the flag on the system which led to a failure in upselling the new offer. I see "failure" is unprofessional in this context.

Can I say the following instead:

"led to unfulfillement in upselling the new offer"

You want them to do something that they failed to do. What's unprofessional about that?

You can, but many will groan at the ugliness of the phrasing.

I find it strange to call the person in Customer Care the agent.

Failure is what it is - calling a spade a spade.
