    Post the dame

I am applying for a job where the headmaster is a reverend. i have two questions about this: firstly, in the address do i address him as 'The Reverend J. Bloggs', 'Reverend J. Bloggs', 'Rev. J. Bloggs' or just 'Mr Bloggs'? followed by 'Headmaster' and then the address? Secondly, when starting my letter how do i address him then? Dear Rev. J. Bloggs, Dear Reverend J. Bloggs etc?
    Thank you.

In the address, Rev. J. Bloggs will be quite normal, with Headmaster on the following line; start the letter Dear Mr. Bloggs
