    Then / Then,

    Dear Teacher

    Then, / Then Mrs Smith took off her high-heeled shoe and killed the centipede.

    Should 'Then' in the above sentence be followed by a comma?

    It can be, but often people don't use a comma there. With a single word, the comma appears unnecessary to some, but when using a longer phrase, the comma can make things clearer.
