The first one:-
I would use 'who' but you don't list it as an option. From your list I would use 'that'. Grammatically that is okay because you are talking about a profession rather than a person, but on a personal level I feel uncomfortable using 'that' to talk about a person even when it is indirectly.
I would also move the '1967'. As it stands it sounds as though you are talking about the first transplant in 1967, not the first transplant ever. Better to move it closer to the idea to which it relates.
"The surgeon who, in 1967, performed the first..."
The second sentence doesn't need anything in the first space
"The mistakes I made in my homework..."
but it does need a 'that' in the second space.
