    fruitcake is slang chiefly Brit., means a person considered to be eccentric or insane;
    He acts like such a fruitcake - means that he acts like a fool.
    The fruitcake who escaped from the booby hatch was soon restored to where he belonged; the fruitcake in this sentence means mad, the madman

    There is an expression "as nutty as a fruitcake" - a pun on two meanings of 'nutty' ('containing nuts'/'mad'). Quite recently, 'a fruitcake' has come to mean a madman; at least, recently in the UK - I first heard it in an American film about 30 years ago (The Taking of Pelham One Two Three (film)) - Wikipedia, the free encyclopedia, and I've heard it (only very occasionally) from non-American lips since then.

    In AmE slang, it usually means "you're crazy." In some instances, however, it is also used to describe a very flamboyant gay man.
