    made payable - how should I send the fee?

    The payment is made by a Standing Order to your bank which orders a regular payment to be made - this sentence indicates that the full amount may need to be made up by separate payment. There is not enough context to tell exactly what it intended - the start of the sentence would have helped.

    send the money due [[on/by a cheque that is]] made 'payable to'
